At Our Lady of Perpetual Help Catholic School, our admissions team — led by Maria LaFollette — partners with each family to make enrollment simple, personal, and faith-guided. We walk with you through the process from application to acceptance, helping you access scholarships and financial assistance to make a Catholic education attainable for your family.

Submit an Online Application
Start your child’s OLPH journey by completing our online application. You’ll need to upload:
- A copy of your child’s birth certificate
- Current immunization record
- Most recent report card and standardized test scores (for grades 1–8)
- Teacher recommendation (grades 1–8)
Once your application is submitted, our admissions team will contact you to schedule next steps.
Assessment and Interview
- Preschool, Pre-K, and Transitional Kindergarten: No assessments are required; our team will review readiness through your application.
- Kindergarten and Grade 1: Teacher-led readiness assessment.
- Grades 2–8: Students may be invited to complete an academic assessment and family meeting with the principal.
Admission Notification
After reviewing your application materials, assessments, and teacher recommendations, our admissions committee will notify you of the decision.
Enrollment
If your child is accepted, you’ll receive an email with instructions to complete the enrollment forms and FACTS tuition agreement.
If the applicant has not been accepted, a letter will be sent to the family.
